What is the origin of the phrase "safe and sound"?

"Sound" here means "whole, undamaged". It's not a literary quotation, just a customary phrase. Anybody can see that while it's good to come out of a dangerous situation safe, it's even better to come out of it both safe and sound. (VSD)
