Joel Spolsky, an eminent authority on software development, interestingly notes the dearth of presence of this cumbrous and weighty literary token: autotrephination.

A trephination is an operation that removes a circular section of bone from the skull.
So, and this is just a guess, auto-trephination is doing the operation yourself or via remote control of some sort.
