"Just like downtown"

For at least the last fifty years this saying was and still is used in the Pittsburgh PA area, what does it mean and where did it come from ?

It might possibly have come from an off-color joke, not suitable for this site, whose punchline (in the version I heard) is "Fifteen dollars, same as downtown."
